# Shoulder care starts with simple changes

## Simple changes can help before a procedure

What can you do for a sore shoulder now? Cut back on the task that starts your ache, while still moving the arm gently. Slow strength work can help with many common tendon problems. Surgery isn't the only choice when home care hasn't been enough.

## Home changes can ease repeated strain

Notice whether golf, lifting, or a high reach starts the ache. Do less of that task for a while, yet keep comfortable movement. Stop each slow lift or turn while the motion still feels easy. I'd pick one useful change that I could repeat each day.

A physical therapist can pick movements for better motion and strength. Each week, note sleep, reach, arm strength, and how long soreness lasts. That'll show whether the work is helping with daily life. Your doctor can help choose pain medicine that fits your health and usual medicines.

## A visit checks motion, strength, and the sore area

During a visit, the provider will ask how soreness started, which tasks bring it on, whether you can sleep on that side, and what you've already tried at home. You'll raise and turn the arm while the medical provider checks motion and strength. An X-ray or scan may help after a fall, crash, sudden weakness, or unclear exam. It doesn't replace having the shoulder moved and checked by hand.

Bring any old scan and notes about care you've already tried. Ask the provider whether the sore part seems to be a tendon, joint, or neck. Then ask about the total price and likely number of visits. You'll also want to know how many days or weeks recovery may affect normal tasks.

## PRP and surgery meet different needs

Platelet-rich plasma is the full name for PRP. A clinic draws blood, spins it, and keeps liquid with extra platelets. It doesn't involve surgery, and research on shoulder relief remains mixed. Surgery may make sense for a major new tear or badly worn joint.

Keep up comfortable movement while you decide, unless the injury needs quick care. Write down the daily task you want back and how long you can manage recovery. That'll keep the visit tied to your needs. QC Kinetix has medical providers who examine shoulders and offer non-surgical regenerative treatments, including concentrated PRP made from blood.

1. In the MOON Shoulder prospective cohort of 452 patients with symptomatic, ATRAUMATIC full-thickness rotator cuff tears, physical therapy succeeded in more than 70% of patients at 10 years: only 115 (27.0%) had surgery at any point over the decade. Patient-reported outcomes improved with physical therapy and did NOT decline over 10 years in those who never had surgery. The strongest predictor of early surgery was low patient expectation of physical therapy, not tear anatomy.

2. The August 2025 AAOS Evidence-Based Clinical Practice Guideline on Management of Rotator Cuff Injuries issued 25 recommendations and 4 consensus statements. Among its updates it clearly RESTRICTS the use of platelet-rich plasma and marrow stimulation in rotator cuff repair, limits prolotherapy in full-thickness tears, establishes CT as an adjunctive imaging modality, and endorses early mobilization after repair of small-to-medium tears.

FDA's guidance on the 21 CFR 1271.10(a) criteria of MINIMAL MANIPULATION and HOMOLOGOUS USE is the rule that decides whether a cell or tissue product may be offered as a lightly-regulated 'section 361' HCT/P or requires full drug approval as a 'section 351' biologic. Docket FDA-2017-D-6146; the revised July 2020 version extended the agency's period of enforcement discretion through 31 May 2021 - which has now expired, so products outside 361 need an approved application or an active IND.

FDA states plainly that no stem cell, exosome, stromal vascular fraction, umbilical cord blood, Wharton's jelly or amniotic-fluid product has been approved for the treatment of ANY orthopedic condition - it names osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain and shoulder pain individually. The only FDA-approved stem cell products in the United States are cord-blood-derived blood-forming stem cells for disorders of the hematopoietic system, and there are currently no FDA-approved exosome products.

## An exam can find the sore part

If soreness keeps limiting sleep or daily use, an exam may find the likely cause. Bring an old scan and the questions that matter to you.
